This program provides comprehensive social services with a focus on targeted and intensive case management to at-risk seniors residing in public housing or the community at large.  The ultimate goal of this core program is maximizing each participant’s access to health care benefits and services.  The principal desire outcome is improved health status and increased ability to remain in their own homes as long as possible, thus delaying unnecessary and premature institutionalization and death.
